Communicate with service users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including information technology. 2. Regularly report and effectively communicate service users health status and care aims in care records and verbal reports to Registered Professional Staff members and other members of the multi-disciplinary team. 3. Relate appropriately with service users, carers, colleagues and others at all times maintaining professional boundaries. 4. At all times ensure that own actions, support the equality, diversity, rights and responsibilities of individuals.
