Summary
The Team leader role requires a dynamic and enthusiastic Learning Disability nurse who has experience of working in the community and an understanding of the Transforming Care Agenda and a passion for working with the individual, their carers and networks to support individuals in their own home and explore alternatives to hospital admission To achieve this you will be leading the Intensive support nurses and senior support workers & your role will include Supervision of band 6 nurses & monitoring of band 5 & band 4 supervisions Excellent communication and organizational skills Operational management of IST including annual leave, e-roster leadership of student nurse placements and recruitment Professional responsibilities including CPD & preceptorship. Monitoring of Caseloads and case management to try and help prevent admission. Screening and allocating referrals Charing Nurses meetings and prevention of hospital admissions
