Summary
To provide specialist nursing expertise, facilitating evidence-based, patient-centred care and assessment for patients with Parkinson's disease. To co-ordinate and monitor in-patients / out-patients throughout their assessment and treatment pathway. To work with internal and external MDT and report or escalate patient wellbeing or deterioration in health. To promote and support patients and carers to maintain quality of life balance. To be involved in delivery of relevant programmes of education and development for all levels of staff and to act as a resource for patients and health care personnel.
