Summary
Clinical : Undertake patient facing and patient supporting roles to counsel patients on medication use and ensure effective medication use in line with cost saving targets, QPF and the prescribing incentive schemes as advised by the practice Carry out medicines optimisation tasks Supporting medication reviews and medicines reconciliation, from internal communication and external communication such as discharge summaries and clinic letters as determined by the Practice Support clinical pharmacists in structured medication reviews, provide expertise to address both the public health and social care needs of patients, Manage shared care protocols and liaise with clinical pharmacists for more complex patients. Manage acute medication requests in line with practice protocol Support the Clinical Pharmacist with medicines safety searches and audits Management of practice medication management policies & SPs Technical and Administrative: Support the Practice MDT to ensure efficient medicines optimisation processes are being followed, training and support to others where needed Support national implementation of prescribing policies and guidance within GP practice Support to deliver on QOF and other locally enhanced services and support the surgery in reviewing and developing policies for CQC requirements and DSQS audit and relevant SOPs. All Technicians must work within their competencies and have professional indemnity for their role
