Summary
An important part of this role is supporting service user's transition into the community therefore your role will be imperative on providing care act, funding and liasing with multiple service providers to ensure a safe and effective transition into the community. You will form part of the multi-disciplinary team, and be expected to voice your professional experience to support the patients who are open to the service, and other services that may need your advice or support. Visits to the team or questions are encouraged to appreciate this exciting new challenging role. We encourage staff development, and you will be supported throughout your supervision and appraisal process to develop your skills and reach your potential.
