Summary
Conduct timely home visits, prioritising earlier in the day to support urgent patient needs. Collaborate closely with paramedics to provide seamless, responsive care in the community. Work proactively with our frailest patients to develop advanced care plans, including comprehensive medication reviews. Help prevent avoidable hospital admissions by delivering effective interventions at home. Contribute to reducing overnight hospital stays and conveyance rates to emergency departments. Improve system flow by alleviating pressures on local urgent and emergency care pathways. Deliver high-quality, patient-focused clinical care aligned with best practice and PCN objectives.
