Summary
Proactively identify, assess, establish plan of care to patients presenting with ACS and arrhythmia with the support of the medical team and senior specialist nursing team. Order diagnostic tests and interpret results to take appropriate actions. Make and receive referrals. In charge of inter-hospital hospital transfer list to ensure timely referrals and transfer of care to relevant tertiary centres. Be able to work autonomously within limitations and within a structured MDT with SOPs in place Be able to deliver supported nurse led clinics in response to service needs. Support the team and organisation in delivering innovative services to support patient access to the team and early diagnoses. Assessing and meeting patients on-going care and health and wellbeing needs, develop a care plan and action referrals as a result, ensuring clear documentation in the patients records. Proactively attend ED for assessment of patients with potential ACS diagnoses and oversee their management. Ensure all patients receive appropriate follow-up and relevant information is available to enable patients to make an informed choice about their treatment and care. Take responsibility for management of the Inter Hospital Transfer List for angiogram referrals. To attend MDT meetings, including safety huddles and board rounds. To deliver a seamless service via a MDT approach to care and work collaboratively with other MDTs at Denmark Hill and GSTT to ensure safe and effective transfer of patients. To participate in the identification and development of clinical protocols and strategies to enhance both the continuity and standard of specialist care whilst ensuring equity of access to the service. To participate in the process of operational policies and review annually in accordance with national standards and ensure all care is given in accordance with agreed protocols. To liaise with the CCU nurse in charge to ensure appropriate and timely Admission/discharge/transfers are in place for patients. To perform DCCV with a more competent clinical nurse specialist or a cardiology registrar. To actively involve service users in providing feedback of their experience of the current service and suggestions for improvements. To participate in relevant meetings at Care Group, Trust, Network or National/International Level to represent the service perspective
