Summary
Working collaboratively with the clinical team to support GPs and Nurses in their prescribing Conduct Long-‐term condition Clinics or group consultations Help patients with Structured Medication Reviews (in practice, care homes and for patients who are housebound) Support patients with medication queries, face to face or by telephone Medicine information to practice staff and patients Management of medicines at discharge from hospital, with the aim of helping patients understand their medicines and avoid unplanned hospital admissions Signposting of patients to alternative local services Review and make recommendations for repeat prescribing policies Service development – suggesting and implementing your own ideas Review Medicines safety Implementation of local and national guidelines and formulary recommendations Support medicines related Education and Training Help practice ensure compliance with Care Quality Commission requirements Get involved with appropriate Public health campaigns For Further Details please see the attached document.
