Summary
Libertatem offers complex care services for children and adults who have conditions including brain injuries, spinal injuries, and cerebral palsy. We also care for people with learning disabilities and autism and mental health.

We are looking for an experienced nurse to take on the role of Senior Clinical Lead, covering Maternity leave, managing a small team Clinical Leads across our South region. You will oversee the packages in this area based on clients’ needs and plan your time accordingly.
Principal Accountabilities:
- Delivering personalised client-centred service outcomes, carrying out care assessments, writing care plans, and managing competencies.
- Providing leadership to the Clinical Leads and support staff, positively supporting and working alongside Regional Operations Managers to deliver excellent care delivery.
- Reporting into the Head of Clinical Services and Registered Manager.
- Working with Field Care Managers, directing aspects of their work where it is highlighted as needed.
- Maintaining your professional skills and undertaking required training and development for personal professional growth.
- Performing in accordance with national standards, legislative requirements, relevant regulations, and accepted best practices.
You’ll have the opportunity to:
- Oversee and manage the Clinical Leads, to maintain all required standards and compliance.
- Monitor client and family-centred outcomes, using your expertise to guide and implement changes proactively.
- Support the Head of Clinical Services and the Registered Manager to further develop and add strength to our clinical governance and the services we provide into the community
- Act as a responsive escalation, providing support and advice to the clinical team for them to deliver their role successfully within the care packages to the highest quality
- Develop individualised care plans in partnership with the client, family, and other healthcare professionals, ensuring goals are patient-focused, realistic, and achievable.
- Undertake initial holistic nursing assessments in partnership with the client, their family, and carers, involving other statutory/voluntary/third-party sector providers where necessary.
- Support the region by visiting clients especially where there may be complexity or issue resolution; also observing practise of the Clinical leads when out on visits.
- Engage regularly online and in person with the Regional Operations Managers, who are peers, ensuring a whole holistic approach centred around the clients is achieved.
- Ensure all recording systems and organisational documentation are of a high standard and up to date.
- Motivate and manage your nursing/care team, embedding the company’s values and behaviours and fostering a culture of ‘pride’ in their work.
- Support field-based care support workers and nurses through regular supervisions, competency assessments, and appraisals.
- Report against targets and standards; implement action plans for service improvements that positively impact clients, service quality, and business development.
- Commit strongly to quality standards to ensure compliance with statutory, mandatory, and contractual requirements.
Working with Others:
- Collaborate with Central Support, including HR, to motivate support workers and nurses through informal check-ins, formal supervision, and appraisals.
- Liaising with Care Coordination on the implementation and day to day delivery of rotas, ensuring that field staff are appropriately trained, assessed, and signed off as competent before lone working.
- Promote and protect the welfare of our clients.
- Work in cooperation with members of the multidisciplinary teams to maximise opportunities for our clients.
- Identify information, concerns, and ideas for improvement and share them with the senior management team of Libertatem.
Other duties:
- Seek opportunities for personal and professional growth and development to ensure you remain a thought leader within the sector
- Work within the NMC Code of Conduct, adhering to the code with all aspects of your professional duties, relationships and development.
- Ensure you are abiding by and keeping up to date with best practice and evidence-based research.
- Attend statutory training and any other training sessions as directed by management.
- Be registered with the NMC, to revalidate and seek guidance and support from the Senior team as required with your revalidation in a timely manner.
- Understand the regulatory framework that governs the service that Libertatem provides.
- Be part of the escalation on call rota outside office hours
A Bit About You:
This role is ideal for someone looking to blend clinical nursing skills with coaching and management of people and services. Experience of managing and delivering complex care in the community is essential. Experience with Spinal Injury, Acquired Brain Injuries, Cerebral Palsy; learning disabilities, autism and mental health is also desirable. You should be able to pass on your knowledge to others. You should be comfortable working with both adults and paediatric care; we will provide training where required.
- Professional nursing registration with no restrictions
- Relevant clinical experience – ABI/Spinal Injury/Complex Care/Community Nursing
- Ability to develop effective working relationships key stakeholders such as:
- Case Managers,
- Integrated Care Systems,
- General Practitioners,
- Consultants and
- oother Healthcare Professionals within the multi-disciplinary team
- Experience of People Management and leading teams
- Experience in clinical governance and quality improvement activity.
- Experience of incident and complaint management
- Passion to deliver outstanding care in a rewarding and sometimes emotionally challenging environment
- Excellent communication skills. You are a confident, clear communicator with a flexible and constructive approach to your internal and external stakeholders, and to the team alike and have high standards of written documentation.
