Summary

Patient Care: To provide outstanding care at all times and constantly work to extend clinical knowledge. To undertake comprehensive risk assessments (both clinical and non-clinical) for all patients on the caseload, and to ensure that management strategies are incorporated into all patient assessment and care plans. To inform the RAT Professional Lead of the patients and/ or carers who choose not to comply with prescribed care or who decline to cooperate to resolve identified risks, e.g., patients who refuse to allow a hoist to be used. Full documentation of each case including a completion of the risk assessment should be recorded in the notes. To escalate non-compliance with prescribed care and resolution of identified risk To document all cases and any identified risk To work within available resources to ensure that overall service objectives are met both in terms of consumables and equipment. To be able to assess and manage manual handling risks in unpredictable working conditions. To assess the need for and request appropriate aids / equipment as required. Reporting faulty equipment / equipmemt including the collection from patients home when they are no longer required. To be aware of health and safety regulations, reporting hazards, accidents and incidents and complete the necessary documentation. To assist in the investigations of such incidents. To work to the standards as outlined in the Trusts Infection Control Policy. To advise patients and their cares about the safe disposal of medication (including controlled drugs) in accordance with the Trusts Medication Policy and record this within the nursing notes. Clinical: To provide and maintain an outstanding care at all times, and to maintain and demonstrate clinical competence. To undertake nurse prescribing duties in line with the Trusts Nurse Prescribing Policy and to work towards becoming a registered Independent and Supplementary Nurse prescribing, promoting effective and evidence based nurse prescribing as a RGN. To monitor the work activities and nursing care standards of the team. To plan the allocation of work within the team in order of priority and delegate to the team according to their skills and competencies. To monitor caseloads in conjunction with team members. Discussing with the Team Leader any concerns or significant changes in the case load. Management: To promote good communication within the Planned and Unplanned Care Teams at all times. To maintain and foster effective communication and collaborative working within the Planned and Unplanned Care Teams including external partner organisations. To have well developed communication skills, utilizing tact, empathy sensitivity and reassurance with people who experience sensory loss, dementia, learning difficulties and whose first language is not English.To report any incidents, comments and complaints from clients, carers or relatives to the team leader and Clinical Lead. Following this up with a written report in accordance with Trust policies and procedures utilizing datix. To carry a mobile phone in order to ensure accessibility and prompt response to urgent issues. To maintain nursing records to Trust standards using EMIS and in accordance with Standards for Records and Record Keeping (NMC/HCPC), ensuring that they are kept up to date. Provide records and reports as requested. To discuss any relevant changes in patient's condition or need with the GP and other appropriate agencies in a timely fashion. To co-ordinate team meetings with the support of the Professional Leads and to attend team briefings and to ensure that the information is exchanged and communicated to team members and managers as appropriate. To document all activity data via EMIS within the appropriatetime frames. To be proficient in Microsoft apps. To report absence/ sickness within the team to the Professional Lead / Clinical Lead in line with Trust policies. To support colleagues in the absence of sickness or shortages of staff in other teams, working flexibly across other teams in the absence of the Team Leader. To ensure all available resources are obtained and utilised for the benefit of the patients and the service. To provide clinical guidance to all staff. To monitor and develop their skills and competencies as directed by the Team Leader. To plan and organize clinics as directed. To ensure the effective practice of case management for clients with Long Term Conditions/Care term conditions/care and palliative/end of life care needsTo ensure the effective allocation of resources based on client needs and within the available resources. To enable and encourage independence through education enabling, patients and carers to participate in their programmes of care where possible. To utilize a variety of teaching methods to achieve this. To undertake and contribute to the initial and ongoing assessments of clients To develop within the team effective liaison with the wider Newham MDT. To participate in service development, i.e. care planning meetings. To undertake caseload profiling with the Team Leader and identify the needs of the local population in partnership with other agencies to facilitate proactive working patterns. To work in conjunction with the Team Leader to provide care to patients in line with the Long-Term Condition pathways. Administrative: To be familiar with and adhere ELFTs policies, standards and procedures in relation to service improvement and to ensure that members of the nursing team are aware of the same. To practice to a high standard of professional conduct in line with ELFT policies and NMC/HCPC Code of Professional Conduct, ensuring that other members of the nursing team are working to the same standards. To participate in the development of the primary care health team activities, and act as a key member to take forward service developments in line with the aims and objectives as outlined in local and national initiatives. To participate in quality initiatives, standard setting and audit programmes of care. To participate in research projects as required. To actively suggest and develop changes or adaptations to working patterns that will meet the changing needs of the local population. To participate in service reviews and support the development of service improvements. To participate in and support the development of care programmes which recognizes and promotes the culture of equality and meets the diverse needs of service users. To recognize patterns of discrimination and to take action in line with Trust policies.
