Summary
Responsible for initial patient assessment including:Collection of detailed case history informationInformal assessment and observationFormal standardised assessment procedures with the potential for diagnosis and further assessment, leading to:- An individual programme of care- Referral to other services as appropriate- Communication of assessment and therapeutic findings within themultidisciplinary team , as appropriate- Responsibility of decisions leading to review or discharge of clients Undertake clinical work with individuals and groups of clients. Develop coordinated multidisciplinary programmes of care/care packages. May workindependently or with other SLT colleagues, or as part of a multidisciplinaryteam. Provide advice, strategies and programmes, as appropriate to the widerworkforce, including parents. Manages a caseload with support as determined by department policy,independently seeking support from other specialist colleagues Establish and maintain liaison with parents/carers and other professionals, both within own department and with other services/departments for thecomprehensive managementof clients. Includes attendance and participation at case conferences In all situations, will communicate complex and sensitive information toclients/parents where there will often be significant barriers to understanding.Communication will include both verbal and non-verbal methods Direct and supervise assistants and volunteers in individual and grouptreatment programmes, according to department policy Demonstrate progression of clear therapeutic aims as appropriate to clinicalsetting, with the use of established team outcome measures To collect statistical data and be involved in the use of audit and outcomes to measure and improve quality of care. Contribute to development of procedures, policies and standards for service delivery and service delivery plans Maintain and provide accurate and timely documentation e.g. patient recordsand reports, sharing with others as appropriate, observing data protectionguidelines To record contact electronically within 24 hours of contact Follow best practice SLT standards in accordance with national, regional and professional standards Meet the required level of continuing professional development as stated by RCSLT and required by the Trust. Actively participate in receiving:Individual personal development programme/appraisalPeer reviewReflective practiceClinical supervisionIndependently identifies own development needs and discusses with Line Manager should formal training be required Responsible for ensuring that own practice is safe and appropriate inaccordance with RCSLT and HCPC Provide prospective SLT undergraduates and other professionals theopportunity to observe the work of a SLT. To contribute to the supervision ofstudents Deliver training to other agency personnel, parents/carers, individually and/or in groups on specific SLT related knowledge and skills Attend and provide input at SLT service meetings, multidisciplinary meetings within Health and external multiagency meetings Observes personal duty of care in relation to equipment and resources in course of work and highlights the need for ordering further equipment as required Ensure good time keeping and planning, to execute the role effectively Recognising when support from other specialist colleagues is required, for example when help required in interpreting and analysing assessment results in more complex cases. To refer matters that may impact on service delivery to Line Manager or other authority before taking action To undertake any other duties of a similar nature consistent with theresponsibilities of this post in order to provide a quality Speech and Language Therapy service
