Summary
• To assist Clients with all aspects of personal care. Including, washing, personal hygiene, dressing and assisting with meals
• To enable Clients to participate in daily activities of their choice such as; day trips, recreational/ leisure activities, crafts, reading, writing.
• To be empathic to the needs of Clients and develop warm and trusting relationship with residents to encourage them to express their needs, views and concerns.
• To be able to clearly explain the remit of the service to a range of clients and other professionals.
• To carry out needs and risk assessments, complete support plans, provide support, and carry out reviews, in accordance with policies and procedures and the requirements of stakeholders and commissioners.
• To maintain good links with local statutory and voluntary agencies to enable clients to access appropriate services and support.
• To accurately input all client data onto our electronic monitoring system and ensure records are maintained to evidence key performance targets, and comply with commissioner requirements.
• To support Clients to make informed choices about decisions that affect them and their family, and to take control of their support.
