Summary

Team Leadership and Line Management Line manage the Care Coordinator team, providing day-to-day oversight and supervision, as well as supporting appraisals, and performance management. Support recruitment, induction, and training of new care coordinator staff. Develop team rotas, manage workloads, and monitor KPIs to ensure effective service delivery. Create a positive, inclusive team culture that promotes learning, wellbeing, and high standards. Service Delivery and Operational Oversight Ensure the Care Coordinator team delivers against local and national service requirements, including proactive care for complex patients, enhanced health in care home and social prescribing and personalised care initiatives. Monitor quality, efficiency, and patient outcomes, making service improvements as needed. Act as a first point of escalation for operational issues within the PCN. Support to PCN Management Work with the PCN Manager to implement PCN objectives related to care coordination and wider workforce development. Contribute to service development plans, workforce planning, and reporting to the ICB or other partners. Provide data and insights on performance, capacity, and outcomes to support service evaluations. Stakeholder Engagement Collaborate with general practice teams, social prescribers, health coaches, community teams, and voluntary sector partners. Governance and Compliance Ensure that the team adheres to all relevant information governance, safeguarding, and health and safety policies. Maintain up-to-date records, including the use of digital tools and patient record systems (e.g. EMIS). Support audits, risk assessments, and service reviews.
