Summary

General duties and responsibilities Support people to develop and review personalised care and support plans to manage their needs and achieve better healthcare outcomes. Help people manage their needs by providing a contact to answer queries, make and manage appointments, and ensure that people have good quality written or verbal information to help them make choices about their care. Provide coordination and navigation for people and their carers across health and care services. Helping to ensure patients receive a joined-up service and the appropriate support from the right person at the right time. Support the coordination and delivery of multidisciplinary teams with the PCN. Maintain records of referrals and interventions to enable monitoring and evaluation of the service. Support practices to keep care records up to date by identifying and updating missing or out of date information about a persons circumstances. Refer onwards to social prescribing link workers and health and wellbeing coaches where required and to clinical colleagues where there is an unaddressed clinical need. Actively participate in multidisciplinary team meetings in the PCN. Identify when action or additional support is needed, alerting a named clinical contact in addition to relevant professionals, and highlighting any safety concerns. Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role. Ensuring work is delivered in a timely and effective manner. Contribute to the development of policies and plans relating to equality, diversity and reduction of health inequalities. Clinical System / Medical Records: Confidence and experienced user in -EMIS Utilise EMIS to record all treatment and care, applying all appropriate templates and protocols relating to recording of patient data. Extraction of necessary data from clinical correspondence and input into Electronic Patient Record, i.e., GP2GP and Docman system Ensure full and accurate recording in the patients computerised records. Safeguarding: The post holder will be expected to undertake safeguarding training (Children and Adults) appropriate to their role and adhere to policies and procedures relevant to the area they work in. Confidentiality: In the performance of the duties outlined in this Job Description, the post-holder may have access to confidential information relating to patients and their carers, Practice staff and other healthcare workers. They may also have access to information relating to the Practice and PCN as a business organisation. All such information from any source is to be regarded as strictly confidential. Personal/Professional Development: The post-holder will participate in any training programme implemented by the Practice/PCN as part of this employment, such training to include: Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work. You will be required to keep a record of the patients are seeing for audit purposes
