We directly deliver Mental Health, Physical Health, Pharmacy and Clinical Substance Misuse in an integrated healthcare delivery model to provide effective and responsive care to improve wellbeing and support better outcomes in the future.
HMP Exeter provide a 24/7 healthcare service and shifts required will be both days and nights.
To respond to a wide range of emergency, urgent care, and triage requests within the prison establishment, working independently and in partnership with other clinical grades of staff. Technicians will act as a clinical lead for care in the absence of a registered/regulated healthcare professional and will undertake clinical assessment, clinical care, and professional referrals in accordance with the Technician scope of practice and current national guidelines (Joint Royal Colleges Ambulance Liaison Committee, JRCALC) and organisational guidelines/policies/procedures.

The post holder will exercise a high degree of personal and professional autonomy in response to emergency incidents within HMPPS and in relation to these incidents, can plan and reach complex and critical judgments and have decision making skills.
To deliver high quality clinical care within the Integrated Healthcare Service for the prison.
To act as a lead emergency care clinician (in conjunction with registered practitioners where appropriate) when attending a broad range of emergency, urgent and triage calls, providing situational leadership, direction, and direct supervision to junior staff.
To demonstrate and provide robust expert clinical skills with a sound understanding of evidence-based practice to provide a pro-active approach to ensure quality and outcome driven practice on a day-to-day basis.
To participate in managerial and professional clinical supervision programme, training, and annual appraisal and to ensure that it becomes an integral part of team practice.
Practice in accordance with the Organisation values and uphold professionalism as a healthcare professional, promoting positive patient experiences and striving for outstanding patient care.
The post holder will work collaboratively with the Clinical Lead, GPs, nurses, and other clinicians to meet and review service needs as required.
The post holder will be part of skilled multidisciplinary teams comprising all healthcare staff, including strategic partners, and prison managing a single referral pathway.
To liaise with other senior Trust personnel to share expertise, promote ideas and to ensure the service is working collaboratively with the Trust's other Directorates as required.
To work positively and effectively in a difficult and often hostile environment.
Be aware of environmental hazards in the working area; ensure staffs are aware of relevant health & safety policies deriving from the Health & Safety at Work Act.
To work in accordance with Trust policies and guidelines.
To be familiar with Prison Standards, Standing Orders, Circular instructions, Notices to Staff, Health and Safety and all other relevant material to function as an informed practitioner within a prison setting.

Clinical
To support the delivery of an effective integrated healthcare service clinical model.
Undertake diagnostic tests, specifically vital signs (respiratory rate, pulse rate, blood pressure, temperature, oxygen saturation), blood glucose monitoring, and 4/12 lead ECG.
Be able to interpret the results in line with training, experience and using decision support tools to ensure the appropriate treatment pathway is used for the patients presenting condition.
To make an informed decision as to when to request further advice from another appropriate clinician.
To manage and maintain the pre-hospital emergency care equipment, ensuring appropriate checks are completed.
Implement safe and effective manual handling techniques to safely move and handle patients, thus reducing risk of injury/incident and promoting safety and comfort.
Use conflict resolution, disengagement and de-escalation knowledge and techniques during conflict situations in accordance with Trust training.
Ensure all consultations are delivered in a clinically effective and timely way, in line with service needs.
Practice legally and ethically and assess/document mental capacity and consent for all patients, making best interest decisions in the absence of mental capacity.
Practice within limits of own competence and area of responsibility and refers any issues to a relevant practitioner.
Ensure the timely, accurate and legible completion of documentation including but not limited to clinical records, work related statements and prison documentation, ensuring records are legible and use professional terminology. This should include the reporting of clinical risks/incidents through the incident reporting system.
Maintain patient confidentiality, in line with NHS standards and the Caldicott principles, including but not limited to professional discussions, clinical documentation and the security of confidential information.
Support patients and colleagues through effective communication and signposting when frequently dealing with highly complex and distressing situations such as end of life care, sudden death, and traumatic incidents.
To support the development, implementation, and maintenance of the National Standards for Health Care in Prisons for the service.
To maintain close working partnerships with all services that may be involved with the care and management of offenders, (prison staff, Ministry of Justice, Trust & Directorate senior managers & relevant forums, CMHT staff, NHS Hospitals, Probation Services, CPS, Courts, GP's, community health services, Housing/Benefit services, social care, community multi- agency forums etc).
IMPORTANT INFORMATION, PLEASE READ:
All applicants must be willing to undertake National Security Vetting in order to work in a Prison Setting. This will be completed as part of the pre-employment checks through Oxleas and the prison vetting team.

Proof of ID, needs to include 1 photographic ID
Proof of address documentation
Non-UK passport holders will need to have correct documentation (right to work in the UK) and a Home office Share code.
Address History:
5 years address history will be needed.
Applicants that are not UK Passport holders who provide less than 5 years UK address history will need to provide a Police Certificate which must be in English from where they resided previously.
Applicants who are UK Passport holders who have lived abroad for a period of more than six months during the last three years will need to provide a certificate of good conduct or an overseas police check in English from the countries resided in or visited.
