Summary
To play a key role in receiving and managing referrals made to the hospice. To ensure a timely, responsive, and effective service for the benefit of all stakeholders involved in the patient referral. Manage all aspects of the referral system to the hospice identifying appropriateness, urgency, and level of priority. Work collaboratively with internal and external partners to support the coordination and management of patients referred to the hospice, and ensure care is planned to meet the individualised and dynamic needs of the patients, and those close to them. Use assessments and interventions as opportunities to train, coach and mentor clinical staff and visitors to the hospice about specialist palliative care practice. To provide immediate and expert specialist nursing assessment, advice and care working alongside existing care providers, and referring to any new services which will benefit the patient and or their family. Facilitate and complete Advance Care Planning (ACP) discussions, ensuring that patients' wishes are clearly documented and communicated in accordance with legal and ethical standards. Plan, organise, and run regular clinics focused on [e.g., end-of-life care, symptom management, and update/facilitate Advance Care Planning (ACP) discussions, ensuring timely access to specialist support and continuity of care.
