Summary
Promote self-care with health education when visiting patients and their carers/family in order to increase understanding and promote maximum independence. Undertake Specialist Nursing and Continuing Care assessments (with appropriate training) The ability to work independently and as part of a team, managing own clinical caseload on a day to day basis Ensure confidentiality of patients and their care is maintained, demonstrating knowledge of principles and purpose of data protection - Record, retrieve and access information in various formats An ability to provide and receive complex, sensitive and contentious information: using empathy and reassurance skills Can provide motivational and negotiation skills in areas where consent/ co-operation is required or there may be barriers to understanding.
