Key Task and Responsibilities Care coordinate and manage a defined caseload of CYP, families and carers. To co-ordinate & deliver individualised care packages / programmes for young people with first episode of psychosis across the age range up to 18 years old and evaluate these programmes, ensuring high standards of practice in accordance with national guidelines and trust policy. Supporting with the provision of urgent initial assessments of suspected psychosis to meet the 2-weeks access and waiting time standard for first episode psychosis. Support with delivering training across CAMHS teams on identifying and working with young people with psychosis. Provide consultation to other CAMHS staff about working with unusual experiences. Ability to work with complex co morbidities including medical, other mental disorders, neurodevelopmental disorders, and substance misuse. Develop and maintain therapeutic relationships with young people and their families/carer. To assertively outreach difficult to reach young people and encourage participation in treatment as well as facilitating independence in all aspects of daily living. To establish links with community organisations in line with NICE guidance for Management of Psychosis and Schizophrenia in Children and Young People. To be responsible for liaising with young persons professional networks, keeping them informed of developments, changes in risk status and promoting multi agency collaboration. Liaison with Childrens Specialist Services including adolescent inpatient units, social care, educational services, independent sector mental health providers, counsellors and any other professional related to the child care. To take responsibility for the organisation and facilitation of multi-agency meetings. E.g. CPA Meetings, professional meetings, Clinical team discussions for allocated young people. To support young people with the process of transition into adult mental health service when required. To be able to assess and manage high-risk adolescent clients in a variety of settings, conducting risk assessments and formulating safety plans. To be an active part of the GCAMHS duty rota, conducting a holistic nursing assessment and developing risk management plans in collaboration with clients who present to A+E in psychiatric emergency. To effectively and appropriately communicate risks often under difficult to medical staff, other professionals and families involved in the care of the young person. To explain treatment options and effectively monitor medication and lead with liaison with pharmacy and Adult Mental Health Services to support young people on clozapine or receiving depot medication. To support physical health clinic for all young people on the Early Intervention Psychosis Pathway to meet standards in line with The National Clinical Audit of Psychosis (NCAP). This would include physical health monitoring of anti psychotic medication, offering interventions to manage side effects such as weight gain, providing advice and psychoeducation on substance misuse and smoking cessation. To provide Family Intervention for Psychosis. Training and supervision will be offered as part of the post if needed. To work to create an inclusive and supportive environment with equal access to services for young people and carers from Black and Minority Ethnic backgrounds experiencing psychosis. To establish supportive and empowering relationships with the children, younger people and their families and carers. Promoting the recovery of young people and supporting them to build their independence in the community around the transition to adulthood. To support young people to identify early signs of mental health relapse and alert appropriate staff involved in the persons care. To monitor and report any changes, both positive and negative, within the care of the child or the young person, during clinical meetings, supervision and at other relevant times. To act as an autonomous practitioner with an understanding of own limitations and recognition of when to seek support. To maintain accurate clinical records in accordance with professional body standards for record keeping and adhere to the NMC Code of Conduct where applicable. As well as Oxleas NHS Foundation Trust policy and procedures on Information Governance. To be aware of the statutory requirements of both the Children Act and Mental Health Act and use these to inform good nursing practice. To ensure that own practice remains within legal, ethical, and professional parameters and is open to scrutiny from relevant professionals. To ensure the physical health clinic is operating in adherence with good nursing practice guidelines. E.g., wound care, physical health monitoring, and monitoring medication side effects. To liaise closely with the head of profession to promote good, effective and safe practice within the CAMHS nursing establishment and participate in the development and promotion of the nursing role within CAMHS. Management Responsibilities To support with audits, service evaluation and quality improvement projects related to the Early Intervention Psychosis Pathway as relevant. This would specifically include the annual NCAP audit. To act as care coordinator, taking responsibility for initiating, planning and review of care plans and giving consideration to the views of young people, their family, carers, referring agents and other professionals involved in the network of care. This includes organising complex professional network meetings. To implement and contribute to the development, evaluation and monitoring of policies and service development as appropriate. To participate in the recruitment, induction, supervision, appraisal and development of staff, and to conduct formal and informal supervision of trainee nurses. To ensure that work is in keeping with both the Service and Trust policies and protocols. To collect and process relevant statistical data as required by the service. To take responsibility as agreed with the line manager and the service manager for the training and personal development of junior nursing staff. To be clinically accountable to CAMHS consultant nurse through line manager. To represent CAMHS and nursing on Directorate and Trust committees as required and to be able to effectively communicate information relevant to CAMHS. Leadership To take responsibility as agreed with the line manager and the service manager for the training and personal development of junior staff. Provide liaison and consultation to local services on specialist nursing interventions delivered in the team. To contribute to the provision of clinical placements for trainee nurses ensuring that trainees acquire the necessary skills, competencies, and experience to contribute effectively to good mental health care. Clinical To provide high quality specialist assessments, consultations, and evidence based therapeutic interventions for children and young people presenting with Early Onset Psychosis in accordance with Trust and national standards, policies and guidelines. To undertake risk assessment and risk management relevant to individual young person, including protective and risk factors present in the network of significant relationships, and to do this using the knowledge of the multi-disciplinary CAMHS Team. To communicate in a skilled and sensitive manner, information concerning the assessment, formulation, and treatment plans of young people. To ensure and maintain a system of prioritising discussion and prompt response for young people under the EIP pathway who are at greater risk to self and others, and or at greater risk of admission. To practice in a way which is inclusive and considerate of the needs of each individual in the system including family members in their varying developmental stages and current emotional state. To consult to parents/carers (including foster parents) as part of a treatment plan. To work in ways which are sensitive to and appropriate for the needs of families from a wide range of racial, cultural and religious backgrounds. To understand the rights of young people and carers and to assist them in exercising those rights, where appropriate. To take case management responsibilities when appropriate in clinical meetings, giving clear clinical advice about assessment, risk assessment, formulation and treatment for allocated young people. To respond to the needs of young people and their carers in an honest, non- judgemental and open manner, this respects the rights of individuals and groups. To have an up-to-date knowledge of appropriate medication and its management and to be able to assess and give appropriate feedback regarding possible side effects. To have up to date knowledge of physical health monitoring. To take responsibility for the decision to discharge young people from care when agreed treatment programmes have been completed.
