Summary
Proactively identify and manage treatment plans for patients with dementia and serious mental illness. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice. Work with patients in order to support compliance with and adherence to prescribed treatments. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, escalating to senior MHP if necessary. Take prompt action on changes in condition, liaising with other caregivers, members of the clinical (multi-disciplinary) team and/or other agencies when necessary, reviewing care and communicating any amendments to the medical records. To have responsibility for seeking support and advice from General Practitioner or senior MHP/supervisor in respect of issues and difficulties with individual patient care. To attend regular clinical supervision with the senior MHP in order to address caseload management issues and support effective care pathways. Develop skills in respect of specific interventions that are appropriate to the clinical area and act as the lead in developing practice in relation to this. Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care. Occasional exposure to highly distressing or emotional circumstances such as contact with patients who may be terminally ill or their relatives To undertake assessments of individual clients difficulties, establishing risk and determining the appropriate course of action taking into consideration all possible options. The relationship should adopt a partnership approach engaging service users in the positive management of their health and wellbeing. Support the wider PCN teams in the best practice management of mental health. Understand own role and scope and identify how this may develop over time. Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working. Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence. Follow clear referral and escalation mechanisms to meet patient need. Prioritise own workload and ensure effective time-management strategies are embedded within the culture of the team. Work effectively with others to clearly define values, direction and policies impacting upon care delivery. Discuss, highlight and work with the team to create opportunities to improve patient care. Responsible for safe use of specialist practice equipment. Act as a clinical leader within own scope of competence in the delivery of health related services, ensuring that the needs of the patient are a priority. Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role model. Identify own development needs in order to maximise potential. Critically evaluate and review innovations and developments that are relevant to the area of work. To contribute towards to the development of local guidelines, protocols and standards and implementation of these as appropriate. To provide guidance and comments on appropriate policies as required.
