Summary
Liaise with medical services, where appropriate, to ensure service users have access to any necessary medical care upon release.Undertake assessments of service users in prison, to co-facilitate pre-release plans in order to develop comprehensive release plan plans for our service users.To develop links with other relevant agencies that offer aftercare support across England ensuring service users have access to appropriate support post release regardless of their area of release.Be responsible and accountable for the management of small caseload as directed by the Team LeaderTo actively support the CPA process.Jointly facilitate group work with multi-disciplinary colleagues where appropriateTo complete detailed clinical risk assessments at first and subsequent assessment interview, and accurately record and report key risksTo develop care plans in collaboration with service users, carer and other relevant person or agencyTo be conversant with all guidance and operational procedures in relation to offender healthcare at HMP Wormwood Scrubs.
