We are seeking a new Evening and Night nurse to work in an already established team to cover the North Nottinghamshire patients of Mansfield and Ashfield, Newark and Sherwood, worksop and retford.

We are looking for individuals who can be part of our fast-moving, innovative and ambitious team, dedicated to supporting our communities to manage their own health at home as well as giving support to palliative patients and those most vulnerable. The service covers an both the evening shift of 5pm- 11pm, 5pm- midnight , 5pm - 1am and night shift of 7pm-8am.

You will be responsible for providing skilled nursing care to patients across the mid and north Nottinghamshire area. You will also work flexibly with multi disciplinary teams across our service to support patient care.

You will need to demonstrate excellent communication and interpersonal skills and be adaptable, reliable, resourceful and approachable. You’ll need to be organised and confident and able to cope with changing environments as you’ll be working independently in patients homes, support is always available.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies

Our Evening and Nights community team hosts a skilled mixed team of nurses and community support workers. The service works in partnership with the three integrated community day teams who operate from 8am- 6pm to provide a seamless nursing service to patients 5pm- 8am, that put the patient/ service user at the centre of their care and provide effective patient care.

The objective of our evening and nights service is to
• deliver good clinical outcomes for patients in partnership with day services across the whole health community
• to co-ordinate a shared care approach for the patient across primary and community care, so that duplication is avoided and resources are effectively managed
• to provide timely access to a combination of primary and community based services
• to recognise early symptoms of the common causes of illness that often result in avoidance admissions
• to proactively identify patients who are risks or growing risk of hospital admissions or readmission and provide support and care to patients and their families
• Provide support for patients and their families nearing the end of their life, including medicine administration or emotional comfort as well as liaising with the appropriate professional to ensure co-ordination of care.
• Triaging of patients, including but not limited to troubleshooting common complaints, giving worsening advice and escalating as appropriate.

We are seeking skilled nurses to join our Evening and Night Service.

Providing appropriate community nursing care following a robust holistic assessment which enables patients to be cared for in their own homes. You will facilitate the prevention of unnecessary hospital admission/re-admission by reducing the effects of ill health and disability and maximising the individual’s potential for independence.

You will need to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful and approachable. Working hours will be a variety of shift patterns the hours 7pm-8am over seven days including weekends and bank holidays.

Community experience is preferred but those with appropriate transferable skills will be considered. Palliative care experience/ catheterisation skills would be desirable but training can be given if needed to the successful candidate.

The position requires you to work independently however being a team player is vital.
