Summary

The post holder will be practising as an independent specialist clinical practitioner providing patient-centred and evidence-based clinical care, within an agreed scope of competence and practice. The postholder will have specialist knowledge of respiratory conditions requiring on-going ventilatory assistance. They will be able to identify early symptoms of respiratory compromise and will be competent in the management including assisted ventilation. They will provide on-going support to community teams and families so that they may continue to care for individuals at home. This includes the management of complex tracheostomy patients including training & completing tracheostomy changes and assessment, provision & training for patients requiring cough assist devices. The postholder will provide inpatient advice and support to Professionals and family when discussing home ventilation as a treatment option as well as advice and monitoring regarding stabilisation and titration of ventilation, and selection of appropriate equipment for home care. The post holder will work in partnership with the multi-disciplinary team (MDT) to implement and improve the patient pathway and will be expected to develop new skills to further enhance the quality of the service delivered. Additional roles will include day to day leadership and management. The post holder will work within the Motor Neurone Disease (MND) Care Centre. The MND care centre provides a co-ordinated interdisciplinary service to approximately 90 patients in Leeds and the surrounding area. The postholder will join the MND multidisciplinary team responsible for this client group and must be able to work closely with them. Attendance at clinics and team meetings on Thursdays is an important part of this role. Work may take place in hospital settings or in the patients home therefore the ability to travel between locations is a requirement of this role. Key relationships will be with the sleep service, the Respiratory Consultant at the MND Centre, neurologists, the MND Care Centre Co-ordinator, MND MDT, the MND Associations Community Support Coordinator. They will need to work collaboratively with Respiratory Support Workers at national MND Centres to share best practice and have peer support. Please see job description for full details of this post.
