Responsibilities
Conduct comprehensive needs assessments for clients to identify their support needs.
Collaborate with clients to develop personalized care plans tailored to their specific requirements.
Provide guidance and information on available services and resources within the community.
Assist clients in navigating complex social care systems and processes effectively.
Maintain accurate and up-to-date case records and reports on client progress and interactions.
Facilitate communication between clients, families, and service providers to ensure coordinated care.
Monitor and evaluate the effectiveness of care plans and make adjustments as necessary.
Requirements
Bachelor's degree in social work, social care, or a related field.
Proven experience in social care or related advisory roles, particularly with vulnerable populations.
Strong understanding of social care policies, legislation, and best practices.
Excellent interpersonal and communication skills to engage effectively with clients and stakeholders.
Ability to work independently and as part of a multidisciplinary team.
Demonstrated empathy, patience, and a commitment to client-centered care.
Strong organizational skills with the ability to manage multiple cases and prioritize effectively.
