Description
We are seeking a passionate and dedicated ASW - Social Worker with a minimum of 2 years post-qualification experience to join our Independence & Access team. This role is pivotal in supporting individuals to achieve a higher level of independence and improve their overall quality of life. As a Social Worker in our team, you will work collaboratively with clients, families, and various agencies to assess needs, develop effective care plans, and facilitate access to appropriate services. Your expertise will enable clients to overcome barriers and enhance their engagement within the community. You will play an integral role in providing emotional support, advocating for clients' rights, and promoting positive change. The role demands excellent interpersonal skills, a compassionate approach, and the ability to think critically and problem-solve effectively. Responsibilities
Conduct thorough assessments of clients' needs and circumstances.
Develop and implement individualized care plans in collaboration with clients and their families.
Provide ongoing support and counseling to clients to promote independence and well-being.
Work closely with interdisciplinary teams to coordinate services and resources for clients.
Monitor clients' progress and adjust care plans as necessary to ensure optimal outcomes.
Advocate for clients' rights and access to necessary resources and services.
Maintain accurate and up-to-date documentation of client interactions and case progress.
Requirements
A recognized degree in Social Work and full registration with the relevant professional regulatory body.
Minimum of 2 years post-qualification experience in a social work setting, particularly in supporting independence and access.
Strong understanding of social care legislation, policies, and frameworks.
Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
Proven experience in case management and conducting assessments.
Ability to work collaboratively in a multidisciplinary team environment.
Strong organizational skills with attention to detail and the ability to manage a varied workload.
