Summary
The post holder will be responsible for the management and development of their specialist service. They will work with other members of the multi-professional team to provide a specialist nursing service to patients, carers and health professionals by: Attending nutrition MDT clinic and arrange admissions for PEG placements. Counsel patients and carers being considered for artificial nutritional support with regards to: Explanation of procedure Quality of life issues Training requirements Support available Complications associated with treatment. Promote patient autonomy through education and support allowing the individual to make informed choices about their own care. Liaise with endoscopy staff to ensure all correct procedures are followed prior to tube insertions, book patient on list and arrange overnight stay where applicable. To train, support and advise patients and carers receiving parenteral and enteral nutrition. To provide continuity of care for ward discharges and act as a point of contact for patients for trouble-shooting tube related problems. Holistically assess patient suitability for artificial nutritional support enteral and parenteral. Act as an independent non-medical prescriber, in conjunction with the Pharmacists and the multidisciplinary team. Be proficient in the management and care of lines with the potential to be trained to place a PICC line. To conduct training needs analysis, using results to train staff in the practical aspects of delivering safe and competent care to patients receiving nutritional support. The post holder will work with other members of the nutrition team undertaking a consultant led nutrition ward round, as well as independent assessment of patients during the week for both patients on parenteral nutrition and enteral feeding as required.
