The role provides an exciting opportunity to work in a multi-disciplinary setting, with a focus on strengths based practice and promoting independence and recovery.
The main duties of the role include completing Care Act assessments and devising care and support plans, completing reviews; carrying out Mental Capacity Assessments, chairing Best Interests meetings as required, crisis intervention, signposting, providing advice and information and working with people with complex social needs.
There is also a Safeguarding function within the service, which includes completing initial fact finding and assessing and minimising risk.
Our Intermediate Care Service (ICS) is seeking a Social Worker to work within the Reablement Service and support with the waiting list for Care Act assessments or reviews. This role offers a unique opportunity to thrive in a multi-disciplinary environment, collaborating closely with health professionals. We place a strong emphasis on strengths-based practice, crisis management, independence, and recovery, ensuring that each individual receives a comprehensive support tailored to their needs.
The role encompass a diverse range of tasks, including:
- Providing advice and information to service users and their families
-Conducting Care Act assessments and person-centred care and support plans
-Facilitating reviews and Mental Capacity Assessments + Best Interests meetings
-Delivering timely signposting to necessary community resources
-Working closely with individuals with varied health and social care needs, including informal carers
