Summary
The post holder will work mainly under direct supervision from the clinical lead and SPNs. The daily workload will be varied with a range of delegated nursing activities / administration duties but expected:- To work according to the MCHFT Trust Value and Behaviours, supporting members of the Bladder and Bowel team to achieve the service specific and organisational objectives. To provide a continual pathway of support for patients cared for within Central Cheshire Integrated Care Partnership (CCICP) Community. To support prevention of harm to patients with bladder and bowel dysfunction and reduced risks associated with these conditions. To increase quality of life for the patients and reduced stigma related to Bladder and Bowel dysfunction. To provide advice and support to patients and care home staff junior in the delivery of high-quality care and actively support the Community Adult SPN and Clinical Team Lead in the delivery of training and care within CCICP and Care homes in the CCICP footprint.
