Summary

To identify vulnerable groups and ensure patients are supported at home by appropriate services. To act as a resource on the management of the effects of the disease or treatment on the patient, in conjunction with other specialists. To identify and utilise professional and lay resources and facilities at both local and national levels. To ensure that good communication networks are maintained through regular contact with the patients as required.
