Summary
Triage patient online appointment requests, assessing clinical urgency and ensuring patients are directed to the most appropriate clinician or service. Conduct comprehensive clinical assessments, including history taking, physical examination, and diagnostic reasoning. Diagnose and manage a wide range of acute and chronic illnesses in line with clinical guidelines and protocols. Prescribe medications independently within scope of practice and formulary. Order and interpret diagnostic tests, including laboratory and radiology investigations. Develop and implement individualised care plans in partnership with patients, including health promotion and disease prevention strategies. Provide advanced clinical advice and support to patients and colleagues. Maintain accurate and timely patient records using SystmOne Participate actively in multidisciplinary team meetings, clinical governance, and audit activities. Engage in continuous professional development and maintain up-to-date clinical knowledge in line with NMC revalidation requirements. Support practice initiatives related to service development and quality improvement.
