Summary
Dorset HealthCare University NHS Foundation Trust (DHC) highly values the contribution that the CYP Public Health Service 0 – 19 makes to improving outcomes for children, young people and their families, using evidence-based, early intervention strategies.
As a Health Visitor, you will work within the 0 – 19 Children and Young Persons Public Health Service to support the delivery of the Healthy Child Programme. You will work in partnership with children, young people and families, the Local Authority, Early Intervention Services, Local Authority Children’s Services, Early Years Providers, Schools and Education Establishments, voluntary and community sector agencies and other Health Care providers. You will be required to work closely with other members of the CYP Public Health Service 0 – 19, to provide services in response to identifiedneeds, including safeguarding and supporting children, young people and families.
We are looking for an enthusiastic and motivated Health Visitor to join our forward-thinking West Dorset Team, this is a year-round post of 15hrs hours per week. The Teams main base is Bridport Medical Centre, West Allington, Bridport.

You will provide a high-quality person-centred approach to care delivery which always considers people’s safety, privacy, and dignity.
You will work within the Children Young Person Public Health (CYP PH) Service to support the delivery of the Healthy Child Programme and targeted interventions to children 0 to 5 and their families.
You will promote the health and wellbeing of the 0 to 5 population and their families
You will provide specialist advice to others regarding the management and care of patients / service users.
You will devise specialist programmes of care for members of the 0-19 team and other professionals, to deliver and ensure they are delivered appropriately.
You will plan, implement and review health improvement programmes in a range of settings and be proactive in delivering the national and local public health agendas in response to identified health and social need.
You will work in partnership with other agencies and meet agreed standards, objectives, and statutory requirements.
You will provide supervision, mentorship, training and clinical support to students and trained practitioners.
You will provide line management and leadership within the 0-19 team
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
