Summary
The post holder will be a degree educated registered professional, with a sound knowledge of Early Help, understanding of the Family Help model and confident in leading team around the family meetings. You will be confident in working with parents, babies, children and young people who may have communication needs and act as an advocate bringing their voice into their journey whilst an inpatient. This post will support safe discharge home into the families community setting and the assessment of needs to support safe discharge will start from admission, our role in supporting safe discharge will be a thorough assessment of social needs which may impact on discharge as well as liaising with the medical team for new or existing medical needs. You will have a key role in ensuring that community referrals and resources are in place in a timely manner to support families at home. The Family Hub is a new team and as such you will play a key role to support the development and building of the model within Newcastle Hospitals pediatric department and Cardiac department and neonates, this will take leadership and team work skills ensuring that staff and patients are supported through the new initiatives.You must have knowledge and experience understanding safeguarding children and adults process and how to escalate into the safeguarding services. The Family Complex Care Coordinator will lead and manage a familys care using an Early Help Assessment and Planning Framework linking with and drawing upon services/agencies available to families to provide a whole family approach on their journey within their hospital admission and through to discharge home. The postholder will organise complex discharge plans for BCYP with planning commencing from the earliest opportunity with the right interventions and include the complexities of repatriation and transfer difficulties.
