Summary
Responsibilities and key result areas to include:Key Relationships: Multidisciplinary colleagues Community colleagues Physiotherapists and Occupational Therapists working across the Inpatient Integrated Therapy Teams Direct reports: Band 5 rotational therapist Therapy assistant (s) Main Tasks and ResponsibilitiesClinical To autonomously complete the OT process with patients including:o Comprehensive assessment of needo Provide individualised and specialised treatments to patients on your caseload, which will include a variety of physical, cognitive, and psychological conditions.o Lead and facilitate on patient discharges with arranging package of care, providing equipment and community support.
