Summary

To assist the multi-disciplinary team in meeting the needs of children and their families by undertaking duties that support and enable family centred care. To work as an integral part of the multi-disciplinary team. The role includes ensuring that there is appropriate communication with other professionals involved in the care of the Child and Mother. Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot usepublic transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.
