We are delighted to be able to offer a career development opportunity within the District Nursing Service for District Nurse Specialists to progress to a senior, advanced clinical and leadership role. 

Would consider applicants who are coming towards the end of their District Nurse Specialist Practitioner Qualification.

Main duties of the job

You will be involved in developing and delivering the preventative care agenda. You will provide clinical leadership for local development of the national ageing well anticipatory care model in Oxfordshire, working closely with senior leaders. You will build relationships and integrated working with multi-professional teams to deliver the model in local populations.

You will be responsible for ensuring that planned care within District Nursing teams in your area is in line with quality standards and best practice evidence-based pathways. You will ensure that patients on the caseload have the best outcomes and that the caseload is effectively managed.

You will lead the DN teams in the management of patients who have more complex and/or advanced needs.

You will work in an integrated way to provide leadership and support for the locality DN teams with the District Nurse Lead and the locality's District Nursing Locality Lead post holders.

Detailed job description and main responsibilities

Oxfordshire District Nursing Service is forward looking and dynamic, involved in national community nursing initiatives and improving the service to meet future demand.

Career progression is established within the District Nursing Service and continues to be developed, especially around senior clinical and leadership roles.

The District Nursing Service sits within Planned and Preventative Care within the Community Services Directorate and works in an integrated way to deliver care to housebound and other patients in their own homes, clinics and residential care home.

Person specification Qualifications Essential criteria
  • District Nursing Specialist Practitioner Qualification (DNSPQ)
Personal Qualities Essential criteria
  • Evidence of leading within and across teams, including multi-disciplinary and cross organisational agencies
Qualifications Essential criteria
  • Advanced History taking or Comprehensive assessment of the person with frailty module
Qualifications Essential criteria
  • Non medical prescribing (V300) qualification, or willingness to undertake
