Further information is available in the attached job description and person specification Work within the Urgent Community Response Team, providing high quality, caring, compassionate care to patients and their families/carers. To include care as directed by UCR clinician following bespoke care plan. Work under the direction of the Registered Healthcare professionals and senior colleagues, both independently and as part of the multi- disciplinary team. Under the direction of a Registered Health Care Professional ensure a high standard of service provision for patients in community, by practicing effective clinical and administrative preparation, effective communication and care delivery, prioritising workload appropriately and contributing to service improvements Participate in giving compassionate high quality care to patients prioritising appropriately and developing competencies that safeguard practice. Develop competencies appropriate to the role. Act always in a manner, which illustrates respect for privacy, dignity, equality, diversity, and confidentiality. Exhibit professional attitude and behaviour and always demonstrate excellent customer care skills. Contribute to effective team working by being fully involved, committed, and participating, to achieving own and others full potential. The role will include visiting patients in their own homes undertaking lone working without direct supervision. The scope of the role will focus on supporting admission avoidance, early supported discharge, promotion of independence and assisting in the effective management of patients whilst on the Urgent Community Response caseload. Will be required to cover other Band 3 work within the neighbourhood team or Stockport borough during periods of annual leave, sickness and maternity leave as appropriate for the maintenance of the service. If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
