Summary
Lead Responsibilities To be agreed Clinical Responsibilities Home Visits Attend scheduled and urgent home visits for housebound or vulnerable patients. Conduct comprehensive assessments in the patient's home environment. Manage acute illness presentations and chronic disease exacerbations in the community. Identify safeguarding or social care concerns and liaise with appropriate services. Document visit outcomes clearly in the medical record. Escalate concerns to duty team or patient's own GP when necessary. Clinical Responsibilities Clinics Conduct face-to-face and telephone consultations for acute and chronic conditions, and minor illness Take detailed histories and perform clinical assessments independently. Diagnose and treat a wide range of conditions within the scope of paramedic practice. Initiate and interpret diagnostic tests (e.g., ECGs, bloods, urine tests). Prescribe or recommend appropriate treatments and medications (if qualified to prescribe). Make referrals to GPs, nurses, or secondary care when outside scope of practice. Provide patient education and health promotion advice. Document consultations accurately in patient records using clinical systems SystmOne). Participate in regular case reviews and multidisciplinary team meetings. Professional Duties Maintain professional registration and adhere to regulatory standards. Participate in continuing professional development (CPD) and clinical supervision. Follow local clinical pathways, protocols, and prescribing guidelines. Contribute to service audits, quality improvement, and practice development. Support induction or training of junior paramedics or students (if applicable). Work collaboratively within a multidisciplinary primary care team.
