Crynodeb
You will be responsible for dietetic assessment, treatment planning and goal orientated interventions to adults referred to the service within a variety ofCommunity settings such as clinics, group sessions, care homes and domiciliary visits. This may also be delivered via phone and video appointments and virtual groups. Participate in multi-professional working for the purpose of dietetic care You will hold an individual caseload You will maintain record keeping in line with professional and trust guidance You will be responsible for your own clinical professional development and HCPC registration You will participate in mandatory training as required by the Trust You will actively participate in a regular clinical and managerial supervision and the preceptorship programme to support your ongoing professional development You will act as a resource on nutritional care to other colleagues in the community multidisciplinary team You will have the opportunity to contribute to audits and quality improvement projects within the team Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
