We are looking for a new Health and Behaviour facilitator to enhance our offer to the people we support and ensure that we tackle health inequality face on. We are looking for someone who has robust experience in this area as the role sits within our higher level 4 management banding. You will need to have a passion for empowering people to lead an aspiring life, by ensuring that people have the right access to health care, are supported with screening, and given the information required to make decisions about their own health. The new role is an exciting opportunity to make real change, impact and challenge the current status quo in primary care.

You will need to be kind, patient, resilient, compassionate, proactive, and believe strongly in great leadership. The role can be tough, but the rewards outweigh, many of the individuals we support face daily discrimination and are at higher risk of health mortality this role can make a real difference.

You will need to be adaptable, flexible, and be able to lead staff to support our people to gain back their independence through empowerment, active support, positive risk-taking and out of the box thinking.
