Summary
Assessing and triaging patients, including same day triage As appropriate provide definitive treatment or make necessary referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols Manage undifferentiated undiagnosed condition and identify red flags/underlying serious pathology and take appropriate action Use complex decision making to inform the diagnosis, investigation and complete management of episodes of care within a broad scope of practice Ensure that patients receive high quality care, delivered in a timely manner Ensure complete and accurate documentation of each patient contact Undertake home visits for patients as required Request and interpret investigations (e.g. pathology, imaging) and act on results appropriately. Support management of long-term conditions in line with evidence-based practice and QOF requirements.
