Summary
holistic assessment, planning, implementation and evaluation of evidence-based care for patients referred to the virtual ward / hospital@home service use remote monitoring and technology as part of care delivery manage clinical events and exercise clinical judgement to manage risk appropriately contribute to the development of the service and quality of care use professional judgement on when to escalate / seek help as appropriate role model the values of the organisation, service and wider healthcare system participate and contribute to new models of care and service redesign identify education and development needs of self and others demonstrate a commitment to research-based practice
