An exciting opportunity has come up for the Adult Community Nursing Service, Night Nursing. We are currently looking for 3 Overnight (33 hours per week) and 2 Twilight (10 hours per week) band 3 Health Care Support Workers to work across the Hounslow areas based at Clayponds Hospital. The service operates from 19:00 until 07:00 seven days a week. The service has 2 shifts, 19:00 until 12 midnight (Twilight) and 19:00 until 7:00 AM (Overnight)

The service also delivers urgent community response between 20:00 and 22:00 as part of the service specification and to prevent hospital admission.

To carry out clinical duties and be prepared to undertake clinical skills as per needs of the service and specific patient after training and competence assessment with support from an identified mentor for example, venepuncture, Blood Pressure, pulse and temperature recording, urinalysis, blood glucose monitoring, simple wound dressings and non-complex tasks such as continence reviews.

To undertake basic duties as delegated and overseen by the registered nurse and actively report any changes in a patient’s condition in a timely way to the registered nurse.

Liaise with other health care and multi-agency workers to provide a multi-disciplinary approach to care.

To communicate with patients at a time and level appropriate to their needs in a manner that promotes positive health.
