Grange and Lakes Integrated Care Community Team, Case Manager, NHS AfC Band 6
30 hrs per week - Permanent
Working Hours: Monday - Friday , 09:00am - 17:00pm

An exciting opportunity has arisen for an experienced Band 5 nurse or paramedic who wishes to progress, or a skilled Case Manager to join Grange and Lakes ICC.
You will provide clinical leadership to a team of Care Co-ordinators, Care Navigators and Admin staff. Your role will involve working closely with the Clinical Team Lead, ICC Clinical Lead, ICC Development Lead, and with other teams from both statutory and voluntary organisations.
You will provide holistic, evidence-based care to complex individuals to promote independence by reviewing their health and social care needs in their home. You will co-ordinate their care, offer clinical advice, and sign-post to additional support. You will manage a complex caseload, collaborating with multidisciplinary teams to develop, implement, and evaluate personalised care plans. Key responsibilities include:
• Supporting independence at home and ensuring high-quality patient care through proactive care planning
• Leading holistic care planning and providing specialist advice to patients and carers
• Acting as a role model and mentor to junior staff; leading by example with sound clinical competence
• Monitoring and improving patient outcomes, including proactive case-finding
• Developing clinical expertise and contributing to service improvement
• Experience:Community nursing experience is highly desirable, especially experience managing a team or willingness to learn. Knowledge of developing person-centred care planning is a must
• Knowledge: A strong understanding of legislations, government policies and guidelines related to older people and their health and social care needs
• Skills:Exceptional communication, both written and verbal, are crucial for interacting with patients, families, and other healthcare professionals. Good interpersonal skills and the ability to build a rapport with people from diverse backgrounds, are also vital. Strong leadership skills required to support the Case Management team
• IT Proficiency: Familiarity with computer systems, databases and IT applications is necessary for managing patient information and data entry.
• Organisational Skills: Ability to prioritise tasks, work under pressure and manage time effectively, are essential for navigating complex patient pathways
• Teamwork and Initiative: The ability to work both independently and as part of a multidisciplinary team, using initiative to problem-solve and find solutions, is crucial
• Personal Qualities:A friendly and approachable manner, a patient-focused approach and the ability to remain calm and objective under pressure, are highly desirable
The Case Manager will be based at Grange Health Centre. Ability to travel across the ICC footprint is expected in order to be able to meet the needs of the local population.
We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
