Summary
To support Medicines Management across all Direct Access Pathways within UHCW To work as part of the clinical pharmacy team to provide a ward-based medicines management pharmacy technician service to include medicines reconciliation, drug history taking, patient counselling, appropriate, timely and accurate supply of medicines including discharge planning, working in accordance with departmental SOPs To undertake clinical verification of oral chemotherapy prescriptions excluding cycle one according to local and national standards following a period of competency-based training and assessment in accordance with local governance processes and policy. To undertake clinical verification of homecare medications excluding initiation prescriptions to local and national standards following a period of competency-based training and assessment in accordance with local governance processes and policy.
