Summary
Provision of clinical expertise to the Hampshire, Sussex and Kent and Provider Collaborative. Ensuring effective care pathways, within and across Provider Collaborative Boundaries. Resolution of complex or disrupted care pathways in conjunction with colleagues across England (and within UK for service users placed in services outside England) Supporting the Provider Collaborative team in ensuring that services within the footprint are compliant to all specifications and Ensuring the delivery of high-quality services that are value for money Ensuring case management systems and record keeping are maintained and kept up to date Maintaining a caseload of young people and supporting their families/carers The person will be required to work as duty within office hours with admin support as part of a rota system with other case managers Attending clinical meetings to support alternative to admissions
