Responsibilities
Provide direct support to individuals in daily living activities, including personal care and household tasks.
Develop and implement individual care plans in coordination with clients and multidisciplinary teams.
Assist individuals in accessing community resources and activities to enhance their social inclusion.
Monitor and report on the progress and well-being of individuals, maintaining accurate records and documentation.
Facilitate communication and build relationships with clients to foster trust and respect.
Support individuals with medication management and ensure compliance with health-related guidelines.
Participate in team meetings and contribute to the development of service improvement initiatives.
Requirements
Minimum of a relevant qualification in health or social care, or equivalent experience in a support role.
Strong interpersonal and communication skills, with the ability to build rapport with individuals from diverse backgrounds.
Proven experience in providing personal care and support to individuals with varying needs.
Knowledge of relevant legislation, policies, and best practices in social care.
Ability to work effectively as part of a team, as well as independently when required.
Strong organizational skills with the ability to manage multiple priorities and workloads.
Flexibility to work various shifts, including evenings and weekends as necessary.
