Summary
Enhance awareness of transition issues, challenges and services available to young people Foster excellent working relationships between Children and Adolescent and Adult Mental Health services. Join a multi-disciplinary team at one of five Tier 3 CAMHS locations Promote mental health wellbeing and physical outcome Promote stability in education and work Reduce episodes of crisis Agreed job plan with specified activity and throughput levels Co-ordinate care for children, young people and families on their caseload who are in process of transition Manage risk within team, Directorate and Trust structures Work to relevant professional and ethical guidelines, and within the framework of Directorate and Trust policies and procedures Contribute to the review and update of protocols for transition between CAMHS and AMHS services Contribute to screening and outcome measures in clinically meaningful ways to demonstrate treatment effectiveness and seek feedback about user experiences
