You will be able to find a full Job description and Person Specification attached within the supporting documents or please click Apply now to view in Trac The Clinical Specialist Physiotherapist within the Stay Well @ Home Team is responsible for the early assessment of patients referred from A&E and the Acute Medical unit in order to prevent inappropriate admissions and enable timely discharge from hospital. They will use highlight specialised, advanced clinical reasoning skills to determine a differential diagnosis whilst taking a major role in the advance assessment of falls risks, identifying further interventions required. They act as an autonomous practitioner, providing expert assessment of the frail older person whilst also providing clinical leadership and operational management for the work of the band 6 physiotherapist and support staff (Across Stay Well @ Home) on the Acute Admission unit. During the absence of the corresponding Occupational Therapist and Principle Social Worker in the team, this post holder will also provide leadership and operational management to the social work and therapy component of the service
