Summary
The following are the core responsibilities of the role. There may be on occasion a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: a) Work as part of the acute / on the day team to manage incoming demand in the practices and, at times, by home visiting or covering ward round. b) Appropriately assess, examine, investigate, diagnose and treat/advise a wide range of on-the-day presentations be these minor illnesses or exacerbations of chronic conditions as part of our acute provision. c) To be professionally and legally accountable for all work undertaken. To practice at an advanced level of professional autonomy and accountability that is within the Professional Codes of Conduct d) Requests investigations such as blood, urine and other laboratory tests, electrocardiographs (ECGs), ultrasound scans, X-Rays and computed tomography (CT) scans in accordance with IR(ME)R regulations and as local policy allows. e) Assesses patients with mental health needs using local policy and guidelines and refers to the appropriate services. f) Ensure patients are informed and consent is gained prior to treatment, investigation and management, as appropriate g) Take responsibility for the management of patients within own caseload and supervision/development of junior members of the multi-professional team. h) Identify the need for appropriate diagnostic tests and interpret the results if appropriate i) Identify and intervene proactively where circumstances contribute to an unsafe environment for patients, staff or relatives and escalate these appropriately j) Share knowledge and expertise, acting as an expert resource to others k) Support and liaise with the wider MDT as needed l) Contribute to the development and improvement of systems and processes that facilitate patients being able to stay in their own homes m) To improve the quality of the patient experience by identifying and meeting the individual clinical needs of patients, including issues pertaining to child protection and vulnerable adults. n) Process and interpret pathology and other test results as required (within the scope of practice) o) Maintain accurate clinical records and ensure read codes are used effectively p) Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice) q) Contribute to practice targets (QOF etc.), complying with local and regional guidance r) Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.) s) Support the clinical team with all safeguarding matters, in accordance with local and national policies t) Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately u) Autonomously formulates appropriate management plans for patients, formulates clinical decisions and often complex treatment plans to manage acute illness and acute episodes of chronic illness including referral, admission, or discharge from service. v) Prioritises health problems and intervenes appropriately in complex, urgent and emergency situations, including initiation and leadership of resuscitation. w) Manages clinical events involving patients, often requiring unpredictable and high levels of mental and/or physical effort according to the patients dependency and clinical need. y)Covid management clinical oversight.
