Summary
The post holder will provide specialised care and support for adults with a learning disability living within the North Tyneside area who present with mental health needs. The post holder will be expected to hold a caseload of service users who may require care co-ordination following discharge from their Mental Health Act Section. The post holder will be responsible for maintaining joint working practices between in-patient team, community social work teams, probation and placement provider staff. The post holder will assist the MDT in the service delivery, be responsible for carrying out comprehensive risk assessments, care plans and support coordinating the provision of therapeutic interventions for people with a learning disability. The post holder will have supervisory responsibilities for junior staff and work within the Integrated Community Learning Disability Team ensuring the service continues to provide the highest possible of care to the patients we support. There is an expectation the post holder will chair, attend and contribute to all relevant meetings and be involved with service development, audits and quality improvements. Car driver essential.
