Summary
Triage, assess, examine, diagnose, plan, implement and evaluate treatments and interventions, integrating both drug- and non-drug-based treatment methods into a management plan Provide information and advice on prescribed or over-the-counter medication accounting for side-effects and potential interactions Develop shared management plans in partnership with patients, to support compliance and adherence to agreed treatment regimes Proactively identify and refer as appropriate, patients at risk of developing long-term conditions Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care Collaborate with clinical and non-clinical support staff.
